The exit gate in Level 0.11 won't open
This is a very rare bug, but sometimes the gate in Level 0.11 becomes completely unresponsive. So far, I have only seen this happen in co-op play.
When this bug occurs, the gate does not respond to the levers at all. Even if all four levers are activated, the gate will not open.
Restarting the run fixes the issue, but because players are often spread out across the level, only the person waiting near the exit can easily confirm that the gate is bugged. As a result, it is often difficult to convince the rest of the team that restarting is actually necessary.
Since this issue is very rare, only people who regularly play co-op may be aware of it, so I doubt this report will receive many votes. Nevertheless, I would greatly appreciate it if you could look into it.

A couple friends have noticed that when completing level 0.11 that the game crashes, we have tried everything from restarting PC, to clearing cache, going solo.. and unfortunately we cannot go any further on the game... -
